A hardware token is simplest way to authenticate and it does not require any network connect to function. It has a built-in algorithm to generate code every time you press the green button. Each token has a unique identification number that is linked to an employee's account so when a passcode is generated it can only be accepted for that user. It also knows when a passcode has been previously used.
